Is anyone aware of a method or process for an offer of street dedication to be accepted and not recorded? If so, how does the information get into the grantor-grantee index?

If it's not recorded, then the info won't make it into the Grantor-Grantee index.

That's why all our resolutions accepting offers of dedication include a clause directing the City Clerk/Clerk of the Board to record a certified copy of the resolution.

In the case of an Irrevocable Offer of Dedication (with a deferred acceptance clause), that can certainly be unilaterally recorded. A well crafted IOD will name the current local agency.

Yes, the method is that the resolution is duly passed to accept the dedication, then everyone goes out for drinks, the resolution ends up in a file cabinet somewhere and everyone goes back to whatever they were working on and the thing never records. The Assessor gets a copy of the resolution and removes the dedication off the tax roll and the copy goes to the 'round file'. Then everyone lives happily ever after until they go to heaven. Then someone tries to figure out where this road came from.

That just about makes sense! It also made my day.

If it's on a map, the following is used by Riverside County and will soon be in use in Alameda County:

The County of Riverside, State of California, hereby approves the Parcel Map and accepts the offer of dedication made hereon for Public Road and Public Utility purposes. The County declares that the acceptance of the offer is to vest title in the County on behalf of the Public for said purposes but that said road shall not become a part of the County Maintained Road System until accepted by resolution of the Board of Supervisors adopted pursuant t0 §941 of the Streets & Highways Code.

We'll change the county to Alameda County, of course...
