Does anyone have a DWG file for the Corner Record form?

The one I used to have dissappeared in the infamous "re-ghost" on my machine.

Here is a zip file containing a Version 2012 .dwg file and our .stb print style file. Yes, I know most of you use the .ctb format, but this is what I have.
